On the morning of Friday, September 16, 2011 the Angel of death came and silently claimed the soul of our beloved Hazel Ann Fields. Hazel was born September 3, 1978 to Alma Fields Moses and Anthony Fields in Ypsilanti, Michigan. Hazel joined and was baptized at Community Church of God in Ypsilanti, Michigan at an early age. Hazel attended and graduated from Bellville High School in 1996 and furthered her education at Michigan State University where she received her Bachelor's Degree in Recreational Therapy. After graduation, she moved to Louisiana where she began her career as a Mental health tech/ Recreational Therapist. She was employed at Quest/ Seaside Behavioral Health in Mandeville, La. Hazel married Larry Moore, Sr. On May 3, 2002 and to this union one child was born, L'Zeah Unique Moore.
Hazel leaves to cherish her memories, one daughter L'Zeah Unique Moore of Covington, La. Her mother Alma (Ray D.) Moses of Angie, La., her father Anthony (Leona) Fields of Wilmington, Delaware, 1 brother Fernando Peters of Pleasant Prairie, Wisconsin. Her fiancee Jason Landor of Covington, La. (Stationed in Iraq) 3 neices; Jada, Jalynn & Julia Peters of Pleasant Prairie, Wisconsin. 7 stepchildren; Jason Landor, Jr. Of Lacombe, La. Marguerite, Larry, Brittany, Micheal, Brandon and Tiron Moore all of Slidell, La., Maternal grandmother, Hazel Peters of Angie, La.: Paternal grandmother Annie Fields of Sunflower, Ms. 4 aunts, Nazea (Sammy) Rogers. Elma (Rev. Charles Pittman) of Angie, La. Brenda Nichols, of Baton Rouge, La. and Linda Toney of Troy, Michigan. 10 uncles, Wilton (Michelle) of Portsmouth, Va.:; Melvin (Debra) Peters of Suffork, Va.; Emeal Peters, Jr. of Akron, Ohio; Odell Peters of Angie, La. Gilbert Fields, Jr. Greggory Fields, Frederick Fields, Marvin Fields and Barry Fields. Her Godmother Penny Bowman of Ypsilanti ,Michigan Hazel is also survived by a host of cousins and other relatives and friends.
She was preceded in death by her maternal adopted grandmother Carrie Mae Mark, maternal grandfather, Emeal Peters, Sr.; paternal grandfather Gilbert Fields, Sr.; 3 uncles, Rodney Peters, Kenneth Fields and Harrison Godbold. Visitation will be Friday, Sept. 23 at Cookrichmond from 2-4p.m. Family hour at Mount Zion B.C. from 5-7p.m. Funeral service will be Saturdday, Sept 24. at Mount Zion B.C. at 3:00 p.m. Dr. A.S.
